Analysis of genetically-related susceptibility to inflammation

Periodontitis is an infectious disease of the periodontium caused by certain bacteria. However, the extent and progress of the disease are largely determined by the response of the immune system to the subgingival microbiome that is present. The level of hereditary inflammatory propensity depends on the number of polymorphisms and the ratio of pro- and anti-inflammatory cytokines which are present. In particular, alterations in the genes of the cytokines interleukin-1α, interleukin-1β, the interleukin-1 receptor antagonist (IL-1-RN) and tumor necrosis factor-α result in a shifted ratio of pro- and anti-inflammatory cytokines.

Patients with a higher genetic tendency to inflammation have an increased risk of periodontal disease. In addition, they are affected by more severe clinical courses than average. At the same time, they show significantly reduced acceptance of dental treatment and significantly more frequent implant complications or losses.

The application of the PadoGen has the purpose of: 

 

  • differential diagnosis in case of above-average disease progression despite inconspicuous microbiological findings.
  • validation of future disease progression (grading).
  • estimation of the course of therapy and treatment planning for periodontitis and periimplantitis.
  • risk analysis before implantations (especially for smokers: higher legal certainty and increase of patient compliance).
  • clarification of familiar predisposition.
  • motivation of the patient to achieve optimal oral hygiene and compliance and to avoid risk factors.

 

In combination with the extent of a patient's subgingival bacterial exposure by the PadoTest® as well as any other risk factors, knowledge of the PadoGen grade enables more informed treatment planning:

 

  • antimicrobial and, if necessary, anti-inflammatory therapy
  • tighter recall program
  • motivation for optimal oral hygiene and compliance
  • avoidance of exogenous risk factors (e.g. smoking, stress, poor oral hygiene)

PadoGen analyzes modifications in the cytokine genes and thus allows an assessment of the hereditary inflammatory tendency of PA patients. The test result represents an important tool for treatment planning and allows a diagnostically supported assessment of individual disease progression.
